Case 1

This is a CT study of a young child, showing marked shortening of the right femur – there is a coxa vara deformity of the right hip; the femoral neck is not ossified, showing a soft tissue bridge between the shaft of the femur and the femoral head with multiple small ossified foci within it. Compensatory sclerosis of the intertrochanteric region is noted.

These findings indicate a congenital developmental abnormality, demonstrating the characteristic features of proximal femoral focal deficiency. I would review radiographs of the rest of the right leg, as this can be associated with other abnormalities such as fibular hemimelia, patellar agenesis and talipes equinovarus.

I would then recommend referral to orthopaedic surgery and discussion at orthopaedic MDT.
