This is a spine MRI study, showing a well defined, small, round lesion at T1/T2 level – the location of this lesion is extramedullary yet intradural, in a midline yet ventral position relative to the cord which it abuts. It shows high signal intensity on the T2 and FLAIR series, is dark on T1, and does not suppress with the CSF. There appears to be communication with the central canal of the spinal cord. There is no enhancement on the post gadolinium series.
This is a cystic lesion, most likely representing a neurenteric cyst. The differential diagnosis includes arachnoid cyst. I would raise this case for discussion at neurosurgery MDT.

What is a neurenteric cyst?
This is a congenital cystic, non-neoplastic lesion which results from displaced endodermal tissue in the CNS during embryological development, forming a simple cyst with epithelial lining.
Question 2:
How may this present?
Patients may present with neurological symptoms such as weakness and paraesthesia due to mass effect on the spinal cord.
