This is a pelvic ultrasound study, showing an enlarged, globular uterus – the cavity is grossly distended and filled with fluid. The fluid shows homogeneous low level echoes; I also note a fluid – fluid level with anechoic fluid rising to a dependent position. The surrounding uterine wall appears thinned due to distension.
I cannot appreciate vaginal or fallopian tube distension on these images.
These findings represent accumulated blood in the uterine cavity, or haematometra.
I would contact the referring physician and advise OBGYN referral.
Question:
What are the causes of haematometra in a pubertal patient?
If there is also haematocolpos, imperforate hymen is the commonest cause. In the absence of haematocolpos, this may be caused by a transverse vaginal septum or cervical / vaginal atresia.
