This is an unenhanced CT brain study showing hyperattenuation in the superior sagittal dural venous sinus, and adjacent cord-like hyperdense structures representing thrombosed cortical veins (cord sign). These features are characteristic of superior sagittal sinus thrombosis. I cannot appreciate any brain oedema or venous infarcts. I would contact the referring physician in the first instance and urgently communicate the findings; I would then obtain a CT venography study to confirm the diagnosis, where I expect to see a triangular filling defect in the SSS representing the thrombus, with an enhancing border representing the dural wall – termed the empty delta sign.
Question:
What are the potential complications of dural sinus thrombosis?
Venous infarcts, which may undergo haemorrhagic transformation; diffuse cerebral oedema; hydrocephalus.
